Event Sanctioning Request Form
All events acting as qualifying competitions; i.e. Association, Regional, Zone, Junior & Senior Championships, etc. (Cost - $110.00)
- OR -
Local & Association events that do not qualify athletes for any further events and any grade level testing. (Cost - $25.00)
Use this form for both Championship and Non-Championship sanctioned event requests. Any event sanction received within 30 days of the event will be issued a late fee.
